The water must be able to circulate and there are three main things to stop this from happening.Heater core blockage,thermostat and water pump.
After checking those you must look at the controls under the dash to see if a dampener door is remaining closed.The doors will shut off the heater core from air circulation when using the air conditioning and vice versa. After starting the van switch the controls for temperature from cold to hot and listen for the door to open and close.That's where I would start.Also you have a Heater Control Valve that shuts off all hot water from entering your heater core.If this valve is closed you would get no to very little heat.It's located in the heater hose from the intake to your heater core.
