The two most common things that cause this are a float in the gas tank or a wiring problem. The float may not be giving a proper signal to the the engine control module (ecm). The float is located in the gas tank and you need to remove the gas tank and fuel pump in order to get a good look at it.
What i would do first is check the resistance of the signal wire that goes from the float to the ecm. If it is at, or less than, .5 mill-amps then the wire is good and your float is not working properly.
