There could be several causes for this. If I were you the first thing I would check is the timing. If the timing is off enough it could cause this. After timing is checked and/or adjusted the next thing I would check the MAP(Manifold Absolute Pressure) Sensor. I can't remeber if a 91 is fuel injected or carburated, but if it is carburated I would check the carb jets and up the idle adjustment just to see how it would run. Do you have a check engine light on in the car? If you do it's more than likely a sensor either the MAP or TPS(throttle position sensor). If you do not have a check engine light on it's got be timing or something else mechanically.

Antilock brake systems don't like fluid pushed back through the system.
You should follow good standard practice, by blocking and ensuring the car is stable and safe.
When the wheel is removed, clamp off the flexible brake fluid line. When you push he caliper back to change the pads, do it with the bleed screw open and catch the fluid that comes out. When the caliper is fully depressed, close the bleed screw up. Change the pads, ensure you put anti-squeel on the outter surfaces. Inspect the clips, ensure they are all seated properly.
Check the runout on the rotor, make sure it's not warped. Get them turned or replace them as necessary.
When done, reinstall everything and remove the clamp.
Start the truck, pump the brakes a few times to get fluid back to the front calipers. Inspect the fluid level in the resivoiour to ensure enough fluid is there.
Do some safe test brakes to ensure all is ok.

A noise can come from anything that moves while the engine is running. What you really need to do is listen carefully from outside the vehicle with the hood open and try and get a better idea where the noise is coming from.
It sometimes helps to use a short piece of old garden hose as a directional listening device...one end on your ear the other end as a probe. Wherever you hear the noise the loudest, is likely the problem or very close to it. If the noise is an internal knocking, likely it is a bearing problem that requires intensive work to correct. Hopefully that's not what you are hearing, Oil pressure good? Serpentine belt tight?

Check your coolant level, low coolant or air in your cooling system will make a "thud"noise in your heater core when the engine is under load from acceleration. The coolant rushes in to the heater core where the air pocket is trapped and then your hear the noise.
Start with the simple and make sure your coolant level is correct and then try to purge the system to remove the air out for your coolant system.

check engine light is on and did the OBD test. Test showed codes for thermostat and sensor, had both of them replaced, and still have same problem as before
