Hello. It seems your truck has either an excessive vacuum leak and/or restricted exhaust problem; this causes rough idle and/or surge conditions. Keep me informed; if you want to.
check that the turbo is fully operational ( variable vanes are free to move , actuator and vacuum hose are in good condition
the smoke is an indication that it is on on the job
the high idle may be from excess fuel and not enough air
and the rough idle and jerking indicates a air fuel ratio problem back again to the turbo
If you have fault codes run them and consider putting it on a chassis dyno for a full test of the pump and turbo
usually cleaning the idle speed contrlol valve from carbon buildup, fixes idle surge and stalling. try soaking vave in throttle body cleaner or inyector cleaner.
it may be a bad tps ( throttle position sensor ). it causes transmission shift issues, bucking,jerking, idle surging, engine cut off, surge while crusing down highway. if this does not solve problem, it may be a sticking valve body in transmission. have a good day !! for an automatic tranny.
Try checking for vaccum leaks, use carb cleaner and spray it around the base of carb, vaccum hoses, edges of intake manifold, take pliers and squeeze the big line to brake power booster & see if it changes, pull the pcv valve out and cover with your finger see if it changes idle.

changed out throttle body gasket and no change... any Ideas would be great. p.s. more worried about high idle then slight fluctuating
