I use to work at a Nissan dealership. It sounds like your are having problems with the torque converter. When the CVT gets replaced, the torque converter does too. They may have gotten a bad part and need to do a parts warrenty on it. Another thing is that they may need to reprogram the TCM. This is another step that should be done after replacing the CVT. If they will not do anything, I would try to find another Nissan dealership and have them look at it.
SOURCE: ENGINE VIBRATION AT IDLE BUT OK WHILE DRIVING.
Could be any one, or more, of the following -
Plugged injector
Bad plug wire
Fouled plug
Crossed plug wires (hard to tell sometimes on V-8s that run smooth anyway)
Vacuum leak
Leaky EGR valve
Shifted timing sensor
Scan engine for codes and proceed from there.
There is a product called RXP (available here in Texas) that will clean up an engine and gas. Its pretty awesome stuff, and this coming from a person who doesn't really believe in additives.

check the drive shaft between the intermediate transmission and the front transmission for miss alignement and defectivness of the telescopics system of this shaft. we had about the same problem on our 1996 same model...
SOURCE: Vibration on acceleration
when some vheel missing weight , the vibration begins at 40-45 mph, now if begins at any moment of acceleration ,check inner drive shafts , no outer joints
SOURCE: AUTO: Vibration in front end of Nissan Altima
the problem could be a bearing on the drive shaft and not hub drive the car and see if u can hear a humming noise for the front also check and see if the front wheel is lose from the center nut if so the bearing in the hub is on the way out . is best to have some one change it as it needs to be pressed out { the old} and in the new . wit the drive shaft on the inner part of it their is an inner joint .this joint can wear and cause the steering wheel to virbate like if the wheels are out of blance not too pricey to do still best to have a pro to do it . best try the steps i provide here to find ur self as 9 time out of 10 u go to a dealer wit one problem and come out wit another . best of luck
