At approx. 90,000 miles temp gauge showed engine was running hot. Coolant levels would be noted to be low and would be topped off 1-2 times a week, which would fix problem temporarily. There was no leakage of fluids noted on the ground, no smell of coolant, no steam from the engine. Only indicator something was wrong was that the temperature gauge would run up to hot, stay up for a few minutes then drop down to the normal range. Sometimes temp gauge would vacillate up and down during drive. No heat inside car until after them gauge would drop back down into the normal range.
These are the things we have done that have not seemed to fix the
problem:
Replaced thermostat- twice, Replaced water pump, replaced heater inlet valve and intake manifold gasket. Bled the coolant system multiple times.
In 12/08 we were driving on the interstate, engine ran hot and this time temp gauge did not drop into normal range. Head gasket blew, warped heads and water was found in the oil. Had engine totally rebuilt by a mechanic. Seemed okay for a couple of weeks but now temp gauge is running up and down again. Have had engine checked out after the recurrence by certified mechanics who cannot find the problem. These mechanics reported excellent work by the original mechanic and did not feel that work done by him was faulty in any way. All gaskets were replaced during original rebuild. Again, no noticible loss of fluids on ground or in engine compartment. No indication of water in oil at this point.

Check your coolant level and see if it is down. If you have checked heads you will notice your engine oil beginning to turn milky white.
If you want a more complete check for cracked heads, plug all spark plugs, and if you can get a coolant pressure tester, pump it up to 12 or 13 psi and then crank the engine over. Watch for water to spray out of one of the cylinders.

There seems to be an air pocket or no antifreeze in engine block. Try this method disconnect upper radiator hose and pour water directly into hose down into engine block until it full. when its filled reconnect hose and fill up radiator. as the vehicle warms up open bleeder valve on the thermostat housing and release air from system. open slowly and BE VERY CAREFUL WATER WILL BE HOT, bleed until you get a steady stream opening and cosing valve. However if all of this don't work. You may have a bad pump. Good Luck

Sounds to me like air trapped somewhere in the coolant's path. You said there's a warm spot on top of the engine. This makes me think of possibly a cracked intake manifold or cylinder head. Did it overheat right before this started? I would have the coolant system 'back flushed' and if the problem is still there, I'd next do a compression test (with the radiator cap off).and check to see if there are air bubbles in the radiator while testing. Make sure the antifreeze is a 50/50 mix and you may want to replace the temperature sensor. The temperature sensor is on the manifold just in front of the thermostat housing and make sure there is no air in the cooling system. Idle the car and place the heater on high to cycle the coolant through the entire system, with the radiator cap on the first click letting any fluid expansion to follow in to the over flow tank and when the engine cools. The radiator will pull the coolant from the over flow tank, once all the coolant has been cycled and no air in the system. Tighten the radiator cap to the second click and that should help you burp the coolant system. As for the temperature running hot could be a faulty sensor, so replace it and it not that expensive and take under 15 min to replace. Good luck friend.

First and foremost. WHERE IS THAT COOLANT GOING YOU ARE ADDING 1-2 times a week. It doesnt evaporate. Have you ever placed the cooling system under pressure manually ? #2 Are the electric radiator cooling fans turning on ?
