It sounds to me like you have a head gasket leaking. See if you can get a gauge and check each cylinder. Are you using alot of anti freeze?

Take that truck into the dealer and have them repair it under the powertrain warranty. 2007 offered a 5 year/100,000 mile warranty.

The smoke on startup is the result of oil getting past the valves.

These trucks, and most GM vehc. Had a problem with intake manifold gasket deterioration. Probably need to replace the intake manifold gasket. Make sure you check the oil also as usually coolant mixes with the oil also. Be careful this can cause seveer bearing damage and catastrophic engine failure. Good Luck.

An EGR that is stuck opencan very well cause the symptom you are decribing, including surging at idle and poor acceleration.

Failure of GM EGR Valves is quite rare.
A common problem on the 4.3L Vortec engines is failure of the Central Multiport Injection Assembly located between the plastic upper intake plenum and the lower intake manifold.

Sometimes failure of injector lines and/or the fuel pressure regulator (Part of the MFI Assembly) will cause fuel to spill into the upper plenum area. This can dislodge carbon deposits in the upper plenum which then get stuck in the EGR Valve and cause the symptoms that you are describing. The EGR Valve can be removed and cleaned. Replacement is not usually necessary.
Ido recommend replacement of the injector assembly however.

the early duramax engines have a problem with cracked injectors, that would be the first thing to check. also keep an eye on your coolant level, could be a head gasket. check the oil for signs of coolant in the oil.

Smoke from tail pipe or smoke from under hood?
A general rule of thumb if it is coming from tail pipe the color of smoke will tell you a lot.

White and thick means head gasket failure, Blue means oil is being burned.

I would recommend you check the seal of everything and make sure it is cleaned that had to do with vacuum or fuel distribution. EGR valve (gasket fails sometimes) IAC valve, MAP and MAF sensors, Fuel injectors, FPR (this will need replaced if leaking),

Also check you coolant and oil look for signs of either one mixing with one another, smell you coolant and make sure it don't smell like gas either, look around where the head meets the block and make sure you don't have any coolant leaking there. The smoke if from the tail pipe makes me think head gasket and these are simple checks. They are not 100% diagnosis but can lead you to the problem.

Also one obvious thing would be make sure no one put anything in your gas tank.

I would most likely look at the injectors first then the head gasket next.

Start the car and it iddles fast and than slow as it does this it smokes out of the tail pipe and than dies.

you have water in gas your going to have to drop gas tank and clean or you can try gas cleaner maybe that will fix the problem.
