An engine piston does not develop a hole for no reason. Pistons are strong and reliable and something else must have happened, something fairly serious, to have caused a hole.
Either the ingress of foreign matter due to bad or incorrect maintenance, something within the engine broke or maybe serious overheating within the cylinder occurred.
Replacing the piston without discovering what caused the trouble in the first place (and curing it) would be a very bad repair.
If it was serious overheating that caused the hole, the overheating could have been caused by preignition or a faulty engine management system and in that case your present problem might be connected to the original cause, though your current problem could be a faulty starter motor or immobiliser.

if you dont trust the mechanic here is a way you can check it for yourelf...look through the hole as a friend tries to start the vehicle for you. if those gears you see dont move when it is cranking over he is right the timing belt broke. he was also right about the possibility it bent the valves which if it did at the very least means a valve job
yes unfortunately when the timing belt goes on your make and model car
the valves will contact the pistons however there is likely only damage
to your valves and not the pistons....so you will want the mechanic to
preform a compression test to confirm valve damage and then go from
there if there is no compression, the valves are damaged and the head
will need to be rebuilt or replaced (when the head is off you can
determine if there is piston damage) in your case with an 11 year old
car you may want to put a used head on it to save money. the timing
belt needs to be replaced every 105,000miles or 48 months whichever
comes first (info received from Haynes repair manual #30034 for

i am curious... what year is this van... and what diesel engine does it have. i am a professional diesel mechanic that specializes on ford engines and i never herd of an R2 engine. it has got to be a 6.2l, 7.3l or 6.0l engine... at any rate none of these engines have a timing belt... unless you have some kind of import model that i am not familiar with. if that is the case them i will try to find out all the info i can.
The cylinder that is missing will indeed set a code, all 96 up cars and trucks sold in the US have this self diagnostic ability, the system is called OBD 2
Yes you are right the repair should be performed for free if it is the same problem, head or head gasket, take this matter up with your local authorities dept of consumer affairs if the shop refuses to help you.
