Every time the van is started there is a loud rattle right off the bat, idles smooth but rattles more loudly the higher the RPMs go. from what I have read it could be a loose heat shield, bad gas, or a bad Cad. converter. any suggestions?

It may be just a exhaust hanger broke loose. Exhaust vibrating against the hanger. IF not then the inside of the converter may be damaged.

Not without knowing exactly where the noise is coming from.
Noise seems to be directly under the gas peddle, can actually feel the rattle when pressing on the ges peddle.
