Because I'm taking my foot off the accelerator and the vibration stops completely, I'm fairly certain that it isn't an issue with the breaks or suspension. I had some transmission work done a little over a year ago, so I'm HOPING it's not the ******. Any insight, ways to diagnose, or places to check would be appreciated!

SOURCE: RPM issue when accelerating

Is this an automatic transmission or a manual shift?
If manual shift,you have a clutch slipping problem.
If automatic,you still have a slipping problem.

2003 RAV4 2WD, 197K, engine/transmission vibration at 45 MPH only.

First record the rpm when the vibration occurs. Then while in park rev the engine to this rpm to see if it is there. If not then the engine should be ok. Next when on the highwaqy and the vibrations starts, put the transmission in neutral and see if it goes away. If so the go faster than when it starts and then put it in N and let the speed back down to 45 and see if it starts again. If it does then you may have a tire balance problem. If it happens while accelerating to 45 and goes away when letting off the gas it may be the u-joints.

Have your Mechanic check the steering tie rod bushings, and linkage. Sometimes it's just an adjustment to the steering box play(located at the end of the steering wheel shaft), - easy to fix.

any car the rpm shouldn't exceed the 4000 round per minutes and you can check it from the sound of the car if it's so loud you should decrease the acceleration pad or give the next gear if it's manual

M,y 2003 rav4 4wd steering wheel vibrates at high speed

the vibration can be your tires, it may need balance, or the tire is out of round. I had seen the tires wear unevenly can cause this. what I mean is because this 4 wheel all the time design, if any of the tire is worn more than the rest can also do that.
the noise is a different thing, it can be a ball joint or tie rod end
