92 paseo 5 speed manual will only shift when car is not running
When car is off I can shift into any gear but when its running it wont let me shift. also if I put it into 1st gear when car is off then try to start it it wants to start in gear the car will jump forward

Sounds like the clutch isnt engaging at all...is there a different sound in engine rpm when car is at idle and the clutch is pressed and released? Check clutch cable.

i had the same problame in my 1989 ranger xlt 4x4 i speed shifted a trans that wasent ment to and my trans got stuck in second and wouldent come out had to take it somewhere to get the forks in the trans replaced turns out i bent the forks and it wouldent come out of second
My wife's 5 speed manual ZX2 has had shifting problems but they are somewhat intermittant. I think it may be the bushings on the shifter rod one pair at the bottom of the shift lever and one where it connects to the transmission shift rod. I can see a good deal of slack motion at the transmission end of the rod by looking at it under the hood with a flashlight while she wiggles the shift lever a bit.
If it is a standard/manual shift transmission, the clutch slave or master cylinder is probably going bad. If you continue to drive it like this it can damage the transmission. I would get these parts checked out ASAP to prevent transmission damage. This is a minor repair compared to replacing the transmission.
If it is an automatic transmission grinding and not shifting, the problem will be inside the transmission itself, and it should be repaired or replaced.
If BVSV is to do with the brake vacuum servo unit, then yes, it could easily be the cause of the high idle speed. This will cause the engine to run lean too, and admit dust and particles into your engine.
First thing to do,try to pump your clutch before you shift into 1st gear then if you feel makes better, most probably you have problem with your hydraulic clutch system, check the clutch line if there is an evidence of fluid leaking or it could be internal leak.
Second thing to do, check your transmission oil, may be it' s too old or you use a thick oil thats why when cold its hard to shift then when the engine rich the normal temperature(warm) the oil become thin and the shifting will smooth. Remember the very old oil become thicken and the purity is gone and can cause of hard shifting. in this regards i suggest you to change the transmission oil and check the used oil if there is small peace of metals, then you are in big trouble,
The throttle position switch has some effect on shifting but should effect both hot and cold. It is normal for Toyota vehicles to lock out the overdrive upshift (3rd to 4th gears) until coolant temperature reaches approx 180 degrees f.
