Engine totally bogs at 2200 rpm code P0204 comes up, changed injector, plugs, still come up

It could be a faulty pcm or wiring as you stated in you post. the pcm pulses a ground to the injector circuit allowing the solenoid in the injector to activate momentarily are inject fuel into the intake. if you give me the engine size. V8 4.7L or strait 6 cylinder 4.0L and i will post the proper wiring diagram for the fuel injectors to help you out with your battle

Check the temperature sensor...it tells the computer when it's cold, and changes fuel mixture accordingly. (same as a choke used to do)
Just curious...If it only starts warm, how do you get it warm???

P0204 is cylinder 4 injector circuit malfunction. Try using a fuel injector cleaner additive to your fuel. After you have used that tank of gas, reset engine ecu and see if it doesnt come back

p0340 is the cam shaft sensor, this will cause it to cut off while driving, hardtime starting, p0351 is the ignition coil A primary/secondary curcuit mal function. p1391 is ignition system misfire, this could be plugs, wires, or coils. first of all have the cam sensor changed and it may correct the rest of the codes. when fixed turn light off with obd2 scanner. if you do not have one, then any auto parts store will erase the codes fo free of charge.
