Quad Driver Module (QDM A) Error The ECM uses "quad drivers" to control its output devices. Each quad driver is capable of controlling up to four devices.
Fault Code Description

• 26 - Quad Driver Module (QDM A) error

Symptoms

• Improper operation of an ECM-controlled device

Common Problems

• Code 26 does not mean there is an internal ECM failure
• Code 26 can indicate a circuit problem (open or shorted) in one of the devices controlled by QDM A
• Some diagnostic charts may state which devices are controlled by QDM A. In other cases, it may be necessary to monitor which ECM-controlled devices are operating when the Check Engine Light comes on in order to determine which circuit is causing the problem.

In that car I have code 26 listed as ""Air/fuel ratio rich". The oxygen sensors are listed as 21 or 27.

26 could indeed be caused by a failed oxygen sensor, but it is more likely to be a stuck cold start injector or a failed coolant temperature sensor.

EOBD II Error Code: 25
Description:
System too Lean, Bank 1
Possible Causes:
- Intake/exhaust leak.
- Secondary air injection (AIR) system.
- Manifold absolute pressure (MAP) sensor.
- Mass air flow (MAF) sensor.
- Fuel pressure/pump.
- Injector(s).
- Evaporation emission (EVAP) canister purge valve.
- Heated oxygen sensor (HO2S).
- Contaminated fuel.

EOBD II Error Code: 26
Fault Location:
System too Rich, Bank 1
Description:
- Evaporation emission (EVAP) canister purge valve .
- Fuel pressure.
- Injector(s).
- Heated oxygen sensor (HO2S).
- Contaminated fuel.

12 battery off, needs 50x start engine,21 o2 sensor,22 coolant sensor,26 check wires on injectors.

Well code 23 is a faulty signal from the manifold air temp sensor. The sensor is telling the computer the air temp in the intake is out of range compared to the other readings.
Code 26 is a quad driver problem inside the computer.
Code 34 is a MAP sensor signal saying the vacuum is high when it should be low compared to the other sensors.
They may be connected or separate issues. You could check the MAP with a vac gauge.
I would clear the codes and see which ones come back again.

Sounds like the ECM, but doudle check your wiring and connections first before comdeming the ECM. The Quad driver is inside the ECM case.

if it is the o2 sensor heater circuit,the car needs to be idled for 2 minutes after cold start then driven for 3 minutes at 60kmh.that will set the monitor.
