Fans for radiator stop working and is overheating

The fans are controlled by the cars powertrain control module through relays, the system can be tested with a scanner for proper function by doing an active test, have this done and see what shows up as a problem. Do not drive this car until it overheats or you will need a very expensive set of head gaskets.

Radiator leaking from the top right corner by the cap metal part

Dry the area completely. Put the cap on. Start it up, look for visual leaks, possibly at the solder/weld. Sometimes a liquid steel or JB Weld would work.

I have a 1996 chrysler town & country that hesitate sometimes when the accelerater is being applied where it's from a stand still or going down the road can anyone help me with this?

HelloThe engine in this cars drive train is fully electronically controlled by a computer called the PCM. Whenever a problem like the computer stores a record of the problem in the form of a fault code in its memory, to read these fault codes you must have the systems memory scanned with a specail tool. Once the fault code(s) are read you then must perform the appropriate diagnostic testing to find and resolve the problem(s)
