My 1993 caprice had the same issue.
Would it happen to down shift out of overdrive on the slitest incline.
if so, one of your plugs is missing under load. Mine turned out to be a wire.

I have a 1991 with 4.3 throttle body injection.

i think you are trying to say the engine will not idle? there are a couple of things that we used to see on these old engines, first the throttle body gasket used to get sucked into the intake and when this happened the engine would idle rough and stall to test for this take a can of carburetor cleaner and spray around the throttle body if there is a gasket leak the engine will rev up when the carburetor cleaner gets into the leak.

also we replaced allot of idle control valves on these models, and i mean a lot.

and lastly make sure the EGR valve is not stuck open, if it is it will cause the engine to stall at idle.

if i were a betting man i would bet on the idle control motor.

let me know if there is any thing else i can do for you.
